Long-term prescriptions: the impact of hospitalisation

In order to assess the impact of hospitalisation on long-term repeat prescriptions - a topic that has received little attention in the literature - we carried out a comparative study of the prescriptions of patients discharged from hospital between 1 March and 30 April 2012. For these 84 patients, the prescription on admission was compared with that on discharge, and these were then compared with the prescription issued by the patient's GP two months after discharge. Our results show that the treatments prescribed on admission were most often continued (17.7 per cent of treatments were discontinued and 7 per cent were changed). Two months after discharge, 77 per cent of discontinuations were maintained and 89 per cent of changes were maintained. Where a medicine was added during hospitalisation, it was continued in 91 per cent of cases two months after discharge. Changes to the prescription that were explicitly stated in the discharge summary were maintained in 100 per cent of cases at the two-month mark. This study demonstrates satisfactory continuity of prescriptions at the hospital-community interface, which is likely further enhanced by communication via the discharge summary.
